Registration Forms

2016-2017 Back-to-School Registration

 

Back-to-School registration is required for each student. Please attend one of the following dates:

Thursday August 11, 2016: 10am-12pm or 5pm-7pm
Tuesday August 16, 2016: 10am-12pm or 5pm-7pm

Each BCS student must be registered before the first day of school. Please contact Nancy Best if special arrangements are necessary.

16. Update of Enrollment Form - 1 form required for each student
17. Emergency and Health Information Form – 1 copies required for each student, sign each copy

18. Student Handbook Signature Page - return one Signature Page for each student
       The 2016-17 Student Handbook is available under Students/Handbook tab.

21. BPS Technology Acceptable Use Policy - return one Signature Page for each student NEW TO BCS

23. Do-Not-Photograph Form - Return this only if you do NOT want your child in BPS media publications.
24. ICHAT Volunteer Consent Form - Each parent/guardian who will volunteer and work independantly with BCS students should complete a form.
25. BPS Pre-Participation Physical Evaluation - interscholastic sports (grades 6-8) or Snow Club (grades 3-8)
